Medical Assessment for Freshmen

READ BEFORE BOOKING!

Before booking, ensure you already have the official results of your Chest X-ray and Drug Test, and a completed Health Record Form.

Submit the following to the Health Services Unit (HSU):

1. Chest X-ray Result (photocopy)
2. Drug Test Result (photocopy)
3. Accomplished Health Record Form (Original)
4. Dental Form
5. Data Subject Consent Form
6. Admission Slip

Reminder:
Be at the venue at least 15 minutes early.
Late arrivals will be served after all scheduled appointments.

Medical Assessment for Internship (COE/ Practice Teaching)

Requirements before booking an appointment:
1. For COE/ internship: Recent Chest Xray result (photocopy)
2. For Practice Teaching/ Field Study: Photocopy of the following:
a. Recent Chest Xray result
b. CBC
c. Urinalysis

Only students with a scheduled appointment & complete requirements will be accommodated.

We provide basic medical and dental services that are essential and plays a key role in supporting a healthy campus environment by conducting medical & dental consultation, physical examinations, health information and health screenings for the RTU community.
